Plugin cache keeps the first 15.7.0 build: rebuilt 15.7.0 with #5190 kept the same version, so refused answers still fall back to Codex

## Before submitting - [x] I searched [existing issues](https://github.com/thedotmack/claude-mem/issues) and confirmed this is not a duplicate. Related: #5189, #5190. ### Bug Description The 15.7.0 plugin installed on Oct 29 does not contain #5190, although the `v15.7.0` tag does. 15.7.0 was rebuilt with #5190 on Oct 30 (`9c4468ebb`, `35d855784`) under the same version number, so `claude plugin update` sees nothing new and the earlier build stays installed. On that build, a refresh answer that claude-mem refuses still goes on to `codex exec`: the Codex quota drain from #5189 continues after the fix shipped. ### Steps to Reproduce 1. Install claude-mem 15.7.0 before Oct 30 17:22 UTC. 2. Update plugins after the 15.7.0 rebuild: nothing updates. 3. Compare `~/.claude/plugins/cache/thedotmack/claude-mem/15.7.0/scripts/worker-service.cjs` with `plugin/scripts/worker-service.cjs` at `v15.7.0`. ### Expected Behavior A rebuild that changes behavior ships as a new version (15.7.1), so installed plugins pick it up. ### Evidence - The installed bundle matches `plugin/scripts/worker-service.cjs` at `1b29d06e7` (chore: bump version to 15.7.0), byte for byte. - `grep -c "not handed to another agent"` gives 0 in the installed bundle and 1 at `v15.7.0`. - Worker log, Oct 31, worker running from the 15.7.0 cache folder: ``` [2026-10-31 11:24:47.880] [WARN ] [WORKER] Agent run (scheduled) failed on claude-code {error=reviews: g_9fb6690dae19: summary is 293 characters; it must be 1–280. Cut at least 13 characters.} [2026-10-31 11:24:47.880] [INFO ] [WORKER] Agent run (scheduled) starting on codex ``` Every Claude Code refusal that day went on to Codex: 10 refusals and 21 Codex runs on Oct 31, 15 and 22 on Oct 30. ### Environment - **Claude-mem version**: 15.7.0 (installed Oct 29, 21:35 CET) - **Claude Code version**: 2.1.316 - **OS**: Linux 6.8 (Ubuntu) - **Platform**: Claude Code CLI, Codex CLI logged in with ChatGPT ### Additional Context The refusals themselves are mostly reviews a few characters over a cap; a fix for those follows in a PR.
