Most AI governance is a document pretending to be an instrument. ServiceNow asked 3,700+ security leaders this month: 76% say they keep a complete, current map of every agent's access. Only 19% can see more than 90% of their agents in real time. The map is what they built. The visibility is what they skipped. A control you can't query is just a belief. Mine were beliefs for longer than I'd like to admit.
